Proximal posterior inferior cerebellar artery (PICA) aneurysms represent a subset of posterior circulation aneurysms that can be routinely treated with either clipping or coiling. The literature contains limited numbers of patients with proximal PICA aneurysms treated with endovascular surgery. On the other hand mirror aneurysms at the origin of the posterior inferior cerebellar arteries represent a highly unusual location. We report a case of mirror aneurysms at the origin of the posteroinferior cerebellar arteries. To the best of our knowledge, this is the second case reported in the literature, whereas is the first treated on both aneurysms with coils.
